Virtual Check-In

Your Safety is our Top Priority. For check-in, we offer two options.

The first option is to come directly into the office for screening/check-in. Please wear your mask, as it is still a federal requirement to wear these in a dental setting. Or, as an alternative, we also offer a “Virtual Check-In”.

To check in virtually, please follow these steps:

1. 1. Upon arrival, please call us at 414-441-6233 if at the Glendale practice, 414-348-5955 if at the Greendale practice, and (414) 247-1990 if at the Fox Point practice.
   2. Please wait in your vehicle.
   3. When you are waiting, we may call to complete a verbal same-day COVID screen, or instead of text or email electronic COVID screening forms if you prefer. If we invite you in prior to screening, we will complete the same-day screen at the front desk.
   4. We will call or text you back when we are ready for your appointment!
   5. Please wear a mask into your appointment. If you do not have one we will provide you with a mask and hand sanitizer.
   6. For check-out, we will help one Patient at a time to maintain social distancing.
